· Why we are able to process your information and what purpose we are processing it for
· Whether you have to provide it to us
· How long we store it for

‘Data controller’ is the term used to describe the person/ organisation that collects and stores and has responsibility for people’s personal data. In this instance, the data controller is Fusion Coaching Limited.
We are registered with the Information Commissioner’s Office; our registration number is ZB096807.
The UK GDPR states that we must have a lawful basis for processing your personal data. There are different lawful bases depending on the stage at which we are processing your data. These are as follows:
If you are currently having coaching or if you are in contact with us to consider coaching or supervision, we will process your personal data where it is necessary for the performance of our contract.
If you have had coaching with us and it has now ended, I will use legitimate interest as my lawful basis for holding and using your personal information.
The UK GDPR also requires us to take appropriate care of any sensitive personal information that you may disclose to us. This type of information is called ‘special category personal information,’ as defined on the website of the Information Commissioner’s Office. The lawful basis for our processing any special categories of personal information is, in addition to the reasons given above, that you have given your explicit consent for me to do so. Therefore, please make sure that you are clear as to the kinds of sensitive personal information that we might process and contact me if you have any questions or concerns.
When you contact us with an enquiry about our services, we will collect information to help us satisfy your enquiry. This will include your name and contact details and a summary of the nature of your inquiry. Alternatively, a professional colleague may send us your details when making a referral, or a trusted individual may give us your details when making an enquiry on your behalf.
We will keep these details in a secure, password-protected location and will not disclose them to any third party without your explicit agreement. If you decide not to work with us, we will ensure all of your personal data is deleted within one month unless you have provided explicit written consent for us to retain and use your personal data for marketing purposes. If you would like me to delete this information sooner, please let me know.
In line with the ethics of the coaching profession, your identity and the details of our work together are confidential; however, there are some limits to this confidentiality. These are listed below.
In the unlikely event that you disclose information which indicates that you or another person is at risk, we will initially discuss this with you with the aim of reducing the risk. However, it might prove necessary to contact another professional. We will always seek to take such a step in consultation with you, and with your consent, but in an emergency our coaches may decide to act promptly for safety’s sake.
Under certain circumstances we are required by law to provide information about our discussions to law enforcement agencies, for example relating to acts of terrorism, money-laundering, harm or potential harm to a minor, or if a member of staff is subpoenaed by a court of law.
During a coaching engagement, we will collect information about you for the purpose of designing and delivering the coaching. Any personal information collected will be stored in password-protected electronic form. Information collected during the course of a coaching engagement will usually be deleted and/or destroyed seven years after the termination of our work. We will not use your personal data for marketing purposes without your explicit written consent.
We try to be as open as we can be in relation to providing clients with access to their personal information. You have a right to ask us to delete your personal information, to limit how we use your personal information, or to stop processing your personal information. You also have a right to ask for a copy of any information that we hold about you and to object to the use of your personal data in some circumstances. You can read more about your rights at ico.org.uk/your-data-matters. If you make a request for access to the personal data we hold about you and we do hold information about you we will:
· give you a description of it and where it came from;
· tell you why we are holding it, tell you how long we will store your data and how we made this decision;
· tell you who it could be disclosed to;
· let you have a copy of the information in an intelligible form.
You can also ask us at any time to correct any mistakes there may be in the personal information we hold about you. To make a request for any personal information we may hold about you, please put the request in writing addressing it to the email address given above.
